Can I upload a file in CSV format?
Yes, you can. The CSV file format uses the same columns as the Excel file; however, it does not expect a header row - only raw data. Also, it expects only the codes for the following columns:
Last Known Residential Setting
Diagnosis
Level of Care Recommendation
Level of Care
Reason Not at Recommended Level of Care
Remove from Waiting List

What line ending characters can I use when I upload a file in CSV format?
You can use any of the following values to terminate a line in your CSV file:
A carriage return
A line feed
A carriage return immediately followed by a line feed

What are the valid codes for the "Last Known Residential Setting" column?
1 = Independent Living (Own Home)
2 = Homeless
3 = Other's Home
4 = Residential Care
5 = Respite Care
6 = Foster Care
7 = Crisis Care
8 = Temporary Housing
9 = Incarcerated
0 = None of the Above
What are the valid codes for the Diagnosis column?
The diagnosis column accepts the following ICD10 Codes

F11.21 = Opioid dependence, in remission
F11.220 = Opioid dependence with intoxication, uncomplicated
F11.221 = Opioid dependence with intoxication delirium

4 = Medically Managed Intensive Inpatient Services
0 = No Treatment Recommended, Assessment Only, or Inapplicable

1 = No Show
2 = Client Request
3 = LOC not available - Referred to another agency
4 = Waiting Period - Referred to another agency
5 = Assessed and Referred
